• Please familiarize yourself on our office and YOUR insurance policy.

• For annual/ well child checks, ensure the visit matches their intended age.

• For multiple children, please call ahead; at this time we prefer to accommodate two at a time.

• Medicaid plans: We are currently not accepting new patients; we are on a temporary hold.
